Revised BNM/APP and BNM/DIR Application Forms and Statutory Declaration Form

This document aims to inform all licensed persons and financial holding companies of the requirement to use the revised application forms and statutory declaration form for the appointment or re-appointment of the Chairman, Directors, and Chief Executive Officer, for submission to Bank Negara Malaysia.

This document should be read in conjunction with the following guidelines:
(i) Guidelines on Corporate Governance for Licensed Institutions;
(ii) Guidelines on Corporate Governance for Licensed Islamic Banks;
(iii) Guidelines on Minimum Standards for Prudential Management of Insurers;
(iv) Guidelines on Directorship for Takaful Operators; and
(v) Fit and Proper Criteria.
